All children regardless of race, creed, sex, or national origin are welcome to attend Child of God Preschool.

In order for a child to be eligible to attend Child of God Preschool, they must be at least two and a half years old, and be fully potty trained.

The following items are required in order to enroll your child:

• Confirmation of the availability of space for the child, and payment of the registration fee.
• Submission of the Child of God Registration Form with parent signature.

Once enrolled, the following must be submitted in order for your child to attend the preschool:

• Completed ODJFS Enrollment and Health Information form. (This form must be submitted by your child’s first day of preschool or your child will not be allowed to attend.)
• Completed ODJFS Child Medical Statement form signed and dated by a physician or certified nurse practitioner. (This form must be submitted within 30 days of your child’s first day of preschool, or your child will not be allowed to attend further. This medical form must be updated annually.)
• (Optional) Completed ODJFS Request for Administration of Medication form if your child is to receive medication or medical treatment while at preschool.

PLEASE NOTE that enrollment may occur throughout the year as spaces become available. There is not just a single enrollment period in late summer / early fall.

Class fees are to be paid by the 1^st of each month. You will be charged a $15 late fee for any class fees not paid by the 10^th of the month.

Registration Fee
There is a $50 non-refundable registration fee for the first child enrolled, and $20 for each additional child enrolled, paid once annually. A child is not considered enrolled until this fee is received.

Supply Fee
There is a $30 supply fee per child to cover the cost of supplies that the preschool purchases in bulk quantities. The supply fee may be paid in two installments: $15 is due by October 10, and the remaining $15 is due by February 10.

There is also a list of basic supplies that the family will need to obtain and bring to class when your child begins school.

Multi-Child Discount
There is a class fee discount of $20 per child after the oldest child registered.
